与颈椎前路手术相关的围手术期并发症: 史密斯-罗宾逊方法

摘要


主要目标是确定低收入国家与颈椎前路手术相关的围手术期并发症的真实发生率。次要目标是确定易导
致这些并发症的任何风险因素;特别是吞咽困难,因为它是最常见的围手术期并发症,并阐明这些并发症的预后,
以及它们的适当处理。这是一项回顾性病例对照研究,包括2018年1月1日至2021年12月15日期间在叙利亚拉塔
基亚的Tishreen大学医院接受右侧颈椎前路介入治疗的109名患者。收集有关性别、年龄、诊断(神经根病或脊髓
病)、手术程序、水平数、钢板添加、吸烟、糖尿病和手术期间发生的围手术期并发症的数据,并在接下来的 14 天
随访中发生。男:女比例为(1.4:1),30.3% 的病例来自一组(41-50)岁。脊髓病占 56%,颈前路椎间盘切除融
合术(ACDF)占47.7%。一级干预在52%的案例中代表最高百分比。围手术期并发症发生率为12.8%;分为吞咽
困难6.4%、喉返神经(RLN)损伤、浅表感染各1.8%、硬脑膜撕裂、压迫性血肿、神经功能恶化各0.9%。钢板加
装、3级或以上干预以及(ACDF P)或(ACCF)手术均被确定为围手术期吞咽困难的危险因素。颈椎前路手术
的大多数围手术期并发症都很轻微,可以保守治疗,预后良好。通过适当的管理及早识别这些并发症对于改善结
果至关重要。钢板加装、3级或以上干预以及(ACDF P)或(ACCF)手术均被确定为围手术期吞咽困难的危险
因素。
