脑钠肽与氨基末端-脑钠肽前体在急性缺血性脑卒中的相关研究进展

摘要


心源性卒中(Cardioembolism,CE)是缺血性脑卒中亚型之一,其病情通常较为严重,具有更高的死亡率,早期及晚期的复发率也更高。心房颤动(Atrial Fibrillation,AF)是心源性卒中的主要病因之一,而脑钠肽(Brain Natriuretic Peptide,BNP)是由大脑、心房或心室分泌的一种多肽激素,具有利尿、血管扩张、抑制肾素和醛固酮的产生以及抗心肌纤维化作用,在诊断心源性卒中方面具有较高的敏感性和特异性。其次部分心房颤动可呈间断性发作,无明显的临床症状(如阵发性、隐匿性心房颤动),临床上诊断较为困难是隐源性卒中(cryptogenic stroke,CS)的重要病因,脑钠肽与氨基末端-脑钠肽前体(N-terminal pro-brain Natriuretic Peptide,NT-proBNP)可识别心房颤动高危患者,减少需要接受心电监护的患者将隐源性卒中病因正确分类,制定更加合理的治疗方案及二级卒中预防措施。
