血浆 NGAL 水平与 STEMI 患者冠脉病变严重程度的关 系

摘要


目的:探讨血浆中性粒细胞明胶酶相关脂质运载蛋白(NGAL)水平与急性 ST 段抬高型心肌梗死患者冠脉病
变严重程度的关系。方法:选择 109 例 STEMI 患者(观察组)和 80 例疑似 AMI 经冠状动脉造影证实冠状动脉正常的患者
(对照组)为研究对象,采集空腹静脉血测定总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密
度脂蛋白胆固醇 HDL-C、血肌酐(SCr)、中性粒细胞计数(N)、白细胞计数(WBC)、超敏 C 反应蛋白(hs-CRP)、
中性粒细胞明胶酶相关脂质运载蛋白(NGAL),采用 SYNTAX 评分系统评价冠状动脉病变程度,并将患者分为轻度病
变组(SYNTAX 评分 ≤22,88 例)和中重度病变组(SYNTAX 评分> 22,21 例)。结果:观察组患者 TG、LDL-C、N、
WBC、hs-CRP、NGAL 均显著高于对照组(均 P<0.05),STEMI 患者 SYNTAX 评分为(15.68±7.69);中重度病变组患
者糖尿病、吸烟比例、N、WBC、hs-CRP、NGAL 均显著高于轻度病变组(均 P<0.05),logistic 回归显示仅 hs-CRP、
NGAL 为冠脉病变程度的危险因素;SYNTAX 评分与 hs-CRP 呈正相关(r=0.526,P=0.001),与 NGAL 呈正相关(r=0.547,
P=0.001)。结论:急性 ST 段抬高型心肌梗死患者血浆 NGAL 水平升高,与冠脉病变程度的密切相关。
