CGF联合Bio-Oss骨粉在外伤性牙脱位牙再植术的临床分析

摘要

目的：探究外伤性牙脱位后行牙再植，于术区牙槽骨周围植入Bio-Oss骨粉，CGF膜覆盖，同时采用牙周纤维带固定松动患牙，观 察其临床疗效。方法：将外伤性牙脱位患者32例，共44颗患牙纳入研究对象，行牙再植术，同期术区牙槽骨周围植入Bio-Oss骨粉，并 覆盖CGF膜严密缝合，分析术前与术后6个月患牙牙槽骨高度、厚度的情况。结果：再植牙术后6个月牙槽嵴顶根方下1mm的颊舌向水 平宽度低于术前牙槽嵴顶根方下1mm的颊舌向水平宽度，术后6个月再植牙远中牙槽嵴顶到釉牙骨质界的垂直距离高于术前远中牙槽嵴顶 到釉牙骨质界的垂直距离，均有统计学差异（P=0.03）。在侧方脱位、嵌入性脱位组中：术后6个月患牙牙槽嵴宽度9.13±0.82(mm)、唇侧 牙槽嵴顶到牙槽窝底高度10.57±2.07(mm)较术前牙槽嵴宽度13.63±1.07(mm)、唇侧牙槽嵴顶到牙槽窝底高度13.66±3.20(mm)减少，术后6 个月再植牙远中牙槽嵴顶到釉牙骨质界高度2.13±0.45(mm)较术前远中牙槽嵴顶到釉牙骨质界高度1.57±0.29(mm)增加，均有统计学差异 （P<0.05）。结论：外伤性牙脱位牙再植存活率较高，虽然术后牙槽嵴高度降低不可避免，但仍然是外伤性牙脱位的首选方法,牙外伤性牙脱 位中，牙侧方脱位和嵌入性脱位是损伤最严重的一种。
