甘油三酯-葡萄糖指数与老年心力衰竭的相关性分析

摘要


目的:探讨甘油三酯-葡萄糖(TyG)指数与老年心力衰竭的相关性。方法:连续收集 2022 年 3 月~9 月就诊于华北理工大学附属医院心内科并依据《2018 中国心力衰竭诊断和治疗指南》诊断为心力衰竭的老年患者设为病例组(n=104),将同期健康体检的老年人设为对照组(n=105),比较两组的临床基线资料、TyG 指数水平的差异。采用单因素及多因素 Logistic 回归分析 TyG 指数与老年心力衰竭的关系。采用受试工作者曲线(ROC)评估 TyG 指数对老年心力衰竭的诊断价值,并确定最佳诊断界值。结果:单因素分析显示,与对照组相比,病例组 TyG 指数水平明显升高,差异具有统计学意义(P<0.05),此外病例组在高密度脂蛋白胆固醇(HDL-C)、甘油三酯(TG)、空腹血糖(FDG)、肌酐、N 末端 B 型脑钠肽前体(NT-proBNP)、左室射血分数(LVEF)值水平、高血压病、糖尿病比例上较对照组相比均具有统计学意义(P<0.05)。多因素非条件 Logistic 回归分析模型显示:TyG 指数(OR=4.068)、糖尿病(OR=3.859)、NT-proBNP 水平(OR=1.006)增加/升高及左室射血分数(OR=0.838)均是发生老年心力衰竭的独立危险因素。TyG 指数对预测老年心力衰竭发生的曲线下面积为 0.824(95%CI:0.768~0.879,P<0.05),最佳诊断值为 9.04,敏感度为 73.1%,特异度为 78.1%。结论:TyG 指数与老年心力衰竭的发病呈正相关,发病时 TyG 指数水平升高,且 TyG 指数对老年心力衰竭的发生具有一定的诊断价值
