智慧健康下老年护理职教创新路径

摘要

目的：在智慧健康背景下，围绕“课程—场景—师资—技术—认证—政策”六维度，构建可复制、可评估、可推
广的老年护理职业教育综合方案，并在城市（上海静安、广州越秀）与农村（江门）多地实施验证。方法： 真实世界教育
前后对照设计；历史对照组 450 人 + 干预组 450 人，共 900 人（三中心各 150），流失率 5%；主要结局为就业率、客观结
构化临床考试（OSCE）胜任力、实习覆盖率、单位培训成本；次要结局为学生与用人单位满意度、12–24 个月就业稳定率。
胜任力指标经两轮 Delphi（5 名专家）确定；统计采用两比例 Z 检验、多变量 Logistic 回归并报告 95% Wilson 区间；同步
建立伦理与数据安全 SOP。结果：组合干预（必修 70h 模块化课程含技术 20h+ 三场景实训 + 师资技术并举 + 分层认证 +
选修 10–15h 不计总学时）后，就业率由 72.2% 升至 91.8%（Δ19.6%，95%CI 10.8–28.4，P<0.01，RR=1.27）；OSCE 达
标率由 58.2% 升至 89.3%（Δ31.1%，RR=1.53）；实习覆盖率由 45.1% 升至 96.0%（Δ50.9%，RR=2.13）。成本 1029 元 / 生·年。
学员满意度 78.5% → 94.2%，用人单位 71.3% → 92.7%；24 月稳定率 >82%。农村实习 Δ52.0%（RR=2.50> 城市 1.85）。 结论：
六维度路径显著提升质量效率，兼顾伦理、经济、可复制。建议省级建统一框架、质量追踪、数据审计。
